H\u00e9patologie <\/strong>– \u00a02005-08-26 –\u00a0COS –<\/p>\n

\nLe diagnostic d\u2019infection du liquide d\u2019ascite (ILA) chez les malades atteints de cirrhose repose sur la d\u00e9tection dans l\u2019ascite d\u2019un nombre de polynucl\u00e9aires neutrophiles sup\u00e9rieur \u00e0 250\/mm3. La pr\u00e9valence de l\u2019ILA semble tr\u00e8s faible chez les malades ambulatoires. Il a \u00e9t\u00e9 sugg\u00e9r\u00e9 que le diagnostic d\u2019ILA pourrait \u00eatre simplifi\u00e9 par l\u2019utilisation de bandelettes urinaires r\u00e9actives (BU). Les \u00e9tudes publi\u00e9es ont \u00e9t\u00e9 limit\u00e9es \u00e0 1 ou 2 centres et ont port\u00e9 sur un faible nombre de patients avec ILA.
\nLes buts de l’\u00e9tude \u00e9taient: a) \u00e9valuer l\u2019incidence de l\u2019ILA dans le cadre d\u2019une \u00e9tude multicentrique prospective, chez des malades hospitalis\u00e9s et ambulatoires ; b) \u00e9valuer la performance diagnostique de la bandelette Multistix 8 SG\u00ae.
\nPatients et M\u00e9thodes
\nDe janvier \u00e0 mai 2004, lors de chaque ponction d\u2019ascite, 2 BU ont \u00e9t\u00e9 test\u00e9es en double insu, conjointement \u00e0 l\u2019analyse du liquide d\u2019ascite chez des malades hospitalis\u00e9s ou ambulatoires, avec ou sans sympt\u00f4me \u00e9vocateur d\u2019ILA. Les cultures du liquide d\u2019ascite \u00e9taient effectu\u00e9es sur des flacons d\u2019h\u00e9moculture a\u00e9robie et ana\u00e9robie.
\nR\u00e9sultats
\n2123 fiches ont \u00e9t\u00e9 collig\u00e9es chez 1069 malades, provenant de 70 centres. 117 infections du liquide d\u2019ascite ont \u00e9t\u00e9 enregistr\u00e9es durant la p\u00e9riode de l\u2019\u00e9tude, dont 56 avec identification d\u2019un germe, et 60 bact\u00e9rascites. La pr\u00e9valence de l\u2019ILA \u00e9tait de 5,5 % dans la population globale: 9 % chez les malades hospitalis\u00e9s, 1,33 % chez les malades ambulatoires, dont 0,67 % chez les malades asymptomatiques.
\nLorsqu\u2019au moins une des 2 bandelettes indiquait une croix ou plus, la sensibilit\u00e9 (Se), la sp\u00e9cificit\u00e9 (Sp), la valeur pr\u00e9dictive positive (VPP), la valeur pr\u00e9dictive n\u00e9gative (VPN), \u00e9taient respectivement de 62,4 %, 95,3 %, 44 %, 97,7 %. Lorsqu\u2019au moins une des 2 bandelettes indiquait 2 croix ou plus, la Se, la Sp, la VPP, la VPN \u00e9taient respectivement de 43,6 %, 99,2 %, 76,1 %, 96,8 %.
\nConclusion
\na) cette \u00e9tude confirme la pr\u00e9valence tr\u00e8s faible de l\u2019ILA chez les malades ambulatoires ;
\nb) la sensibilit\u00e9 et la valeur pr\u00e9dictive positive des BU Multistix 8SG\u00ae sont m\u00e9diocres, alors que la sp\u00e9cificit\u00e9 et la valeur pr\u00e9dictive n\u00e9gative sont excellentes. Une BU n\u00e9gative n\u2019exclut pas le diagnostic d\u2019ILA.<\/p>\n
