But : \u00e9valuer le devenir \u00e0 long terme d\u2019une cohorte prospective de patients \u00e2g\u00e9s de plus de 75 ans explor\u00e9s pour une an\u00e9mie ferriprive (AF).
\nM\u00e9thodes : 102 patients, dont 60 femmes, \u00e2g\u00e9s de 82\u00b15,8 ans et qui ont eu une exploration endoscopie de Juin 2003 \u00e0 Mai 2005 ont \u00e9t\u00e9 suivis prospectivement sur une p\u00e9riode d\u2019au moins 1 an. Les informations suivantes \u00e9taient collig\u00e9es : 1) d\u00e9c\u00e8s (cause et relation avec AF), 2) r\u00e9cidive de l\u2019an\u00e9mie. Une \u00e9valuation (par analyse multivari\u00e9e) de la survie et de la r\u00e9cidive de l\u2019an\u00e9mie a \u00e9t\u00e9 \u00e9tudi\u00e9e dans ces 4 groupes: an\u00e9mie inexpliqu\u00e9e (groupe 1, n=27), l\u00e9sion b\u00e9nigne (groupe 2, n=37), cancers trait\u00e9s carcinologiquement (groupe 3, n=28), cancers trait\u00e9s palliativement (groupe 4, n=10). R\u00e9sultats : Le suivi m\u00e9dian \u00e9tait de 591 jours. Les principales caract\u00e9ristiques clinico-biologiques n\u2019\u00e9taient pas significativement diff\u00e9rentes selon le groupe. 33 (32%) patients sont d\u00e9c\u00e9d\u00e9s dont 10 (30%) le premier mois. 10 (100%) appartenaient au groupe 4 vs respectivement n=7 (27%), n=8 (22%) et n=9 (31%) des patients des groupes 1, 2 et 3 (p<0.001). Aucun d\u00e9c\u00e8s n\u2019\u00e9tait en rapport direct avec l\u2019an\u00e9mie, 5 (15%) \u00e9taient li\u00e9s au traitement. L\u2019an\u00e9mie a r\u00e9cidiv\u00e9 chez 18 (18%) patients (dont 5 (28%) avaient des angiodysplasies et 5 (28%) un cancer trait\u00e9 palliativement) n\u00e9cessitant une transfusion dans 13 (13%) cas. Le taux de survie cumul\u00e9 \u00e0 1 an (n, IC [95%]) des patients du groupe 4 \u00e9tait de 0.1 [0 ; 0.29] significativement plus faible que celui des patients des trois autres groupes \u2013 groupe 1 : 0.84 [0.7 ; 0.98], groupe 2 : 0.86 [0.74 ; 0.98], groupe 3 : 0.78 [0.62 ; 0.94] \u2013 p<0,0001.
\nConclusion : dans une population de personne \u00e2g\u00e9e ayant une AF, le nombre de d\u00e9c\u00e8s et la survie \u00e0 long terme ne sont pas diff\u00e9rents qu\u2019il s\u2019agisse d\u2019une AF sans cause identifi\u00e9e, d\u2019une AF secondaire \u00e0 une pathologie b\u00e9nigne ou d\u2019une AF secondaire \u00e0 une tumeur trait\u00e9e carcinologiquement. Aucun d\u00e9c\u00e8s n\u2019\u00e9tait directement li\u00e9 \u00e0 l\u2019an\u00e9mie.<\/p>\n","protected":false},"excerpt":{"rendered":"
