DI MARTINO<br \/>\nService d\u2019H\u00e9patologie et de Soins Digestifs Intensifs, H\u00f4pital Minjoz, Besan\u00e7on 25030 cedex.<\/em><\/p>\n<p><strong>H\u00e9patologie <\/strong>&#8211; \u00a02008-09-07 &#8211;\u00a0CO &#8211;<\/p>\n<p>________________________________<\/p>\n<p>Le d\u00e9pistage du carcinome h\u00e9patocellulaire (CHC) par l\u2019\u00e9chographie et le dosage de l\u2019alfafoetoprot\u00e9ine permet de r\u00e9v\u00e9ler plus pr\u00e9cocement des l\u00e9sions intra-h\u00e9patiques m\u00eame si l\u2019impact de cette strat\u00e9gie sur la survie reste \u00e0 d\u00e9montrer prospectivement. Patients et m\u00e9thodes : Les donn\u00e9es de 229 patients cirrhotiques ayant un CHC ont \u00e9t\u00e9 collect\u00e9es en 3 ans r\u00e9tro- puis prospectivement; ces donn\u00e9es comprenaient des caract\u00e9ristiques d\u00e9mographiques et cliniques au moment du diagnostic de cirrhose et de CHC et les caract\u00e9ristiques tumorales initiales. R\u00e9sultats : Nos 229 cirrhotiques \u00e9taient franc-comtois pour 85% d\u2019entre eux. Au moment du diagnostic de la cirrhose: \u00e2ge moyen \u00e0 63&#61617;11 ans (extr\u00eames: 26-92), 89% d\u2019hommes, score moyen du Child-Pugh \u00e0 6,4&#61617;1,8 (extr\u00eames: 5-13). Il y avait 37% de diab\u00e9tique (type 2 \u00e0 90%), 77% \u00ab d\u2019alcoolique \u00bb (CDA &gt; 30 g\/jr avec une consommation d\u2019alcool d\u00e9clar\u00e9 moyenne de 82&#61617;56 g\/jr), 11% de NASH ; les autres pathologies \u00e9taient rares : 3 cas d\u2019h\u00e9mochromatose, 2 cas d\u2019HAI et 1 cas de CBP. La cirrhose \u00e9tait d\u00e9couverte lors du suivi d\u2019une h\u00e9patopathie chronique connue, d\u2019une complication (ascite ou RVO), de sympt\u00f4mes (douleurs abdominales ou AEG), d\u2019une biologie h\u00e9patique perturb\u00e9e et fortuitement dans 34%, 21%, 18%, 12,5%, et 12,5% des cas respectivement. Seulement 42% des cirrhotiques ont eu un d\u00e9pistage du CHC, r\u00e9alis\u00e9 par un sp\u00e9cialiste dans 89% des cas (83% d\u2019h\u00e9patogastroent\u00e9rologue, 4% endocrinologue) et 11% par un m\u00e9decin g\u00e9n\u00e9raliste. Un quart des CHC \u00e9tait initialement symptomatique permettant de r\u00e9v\u00e9ler la tumeur avec une AEG, des douleurs abdominales ou un ict\u00e8re dans 72%, 35%, 23% des cas respectivement. L\u2019aFP initiale \u00e9tait \u00e9lev\u00e9e dans 41% des cas seulement avec une valeur m\u00e9diane \u00e0 22 ng\/mL (IQR : 574 ng\/mL) et 31% des valeurs \u00e9taient sup\u00e9rieures \u00e0 200 ng\/mL. l\u2019imagerie h\u00e9patique \u00e9tait presque toujours contributive (94% des cas) pour le diagnostique de CHC. La taille moyenne des l\u00e9sions tumorales \u00e9tait de 6 &#61617; 4,5 cm (range : 1-20 cm) initialement, la taille du CHC \u00e9tait &gt; 9 cm dans 24% des cas et le CHC \u00e9tait infiltratif d\u2019embl\u00e9e dans 10% des cas. Seulement 30% des patients ont pu avoir un traitement curatif (18% de radiofr\u00e9quence, 8% de r\u00e9section et 12% ont \u00e9t\u00e9 transplant\u00e9s). <\/p>\n<p>Conclusion : Les cirrhotiques atteints de CHC arrivent tardivement au CHU de Besan\u00e7on avec des tumeurs d\u00e9j\u00e0 volumineuses emp\u00eachant toute possibilit\u00e9 de traitement curatif. Moins d\u2019un quart des m\u00e9decins g\u00e9n\u00e9ralistes r\u00e9alisent un d\u00e9pistage du CHC chez le cirrhotique. Le d\u00e9veloppement de la TH et l\u2019extension des crit\u00e8res de Milan devrait inciter le corps m\u00e9dical \u00e0 un d\u00e9pistage plus soutenu, surtout dans la population de patients alcooliques et de NASH.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>2008 T.
