(1) CH MEAUX,(2) CH LAGNY,(3) CH COULOMMIERS<\/em><\/p>\n<p><strong>Gastroent\u00e9rologie <\/strong>&#8211; \u00a02010-09-04 &#8211;\u00a0CO &#8211;<\/p>\n<p>________________________________<\/p>\n<p>Le m\u00e9soth\u00e9liome p\u00e9riton\u00e9al malin est une tumeur rare dont l\u2019incidence est \u00e9valu\u00e9e \u00e0 2 par an et par million d\u2019habitants. Il est 5 fois moins fr\u00e9quent que l\u2019atteinte pleurale. L\u2019exposition \u00e0 l\u2019amiante est le principal facteur de risque. Le pronostic est sombre avec une survie m\u00e9diane de quelques mois. Des traitements plus agressifs comportant une r\u00e9section chirurgicale et une chimioth\u00e9rapie hyperthermique intra-p\u00e9riton\u00e9ale ont permis d\u2019obtenir des survies prolong\u00e9es chez certains patients. Nous rapportons ici, 9 cas de m\u00e9soth\u00e9liomes primitifs du p\u00e9ritoine : 7 hommes et 2 femmes d\u2019\u00e2ge moyen  62,1 ans (extr\u00eames : 44 &#8211; 74 ans). Une exposition \u00e0 l\u2019amiante \u00e9tait mise en \u00e9vidence chez 7 patients. L\u2019ascite \u00e9tait le principal mode de r\u00e9v\u00e9lation. Le diagnostic histologique avait \u00e9t\u00e9 port\u00e9 sur des biopsies p\u00e9riton\u00e9ales r\u00e9alis\u00e9es par coelioscopie. Les 9 patients avaient \u00e9t\u00e9 trait\u00e9s par chimioth\u00e9rapie \u00e0 base de sels de platine : cisplatine et Alimta (6 patients), carboplatine et Alimta (2 patients), tomudex et oxaliplatine (1 patient). Une r\u00e9mission clinique avait \u00e9t\u00e9 obtenue chez 5 patients avec cependant une survie globale de 5 mois.
