{"id":1949,"date":"2012-10-01T01:00:00","date_gmt":"2012-10-01T00:00:00","guid":{"rendered":"https:\/\/angh.net\/abstracts\/une-elevation-inhabituelle-dalpha-foetoproteine\/"},"modified":"2018-10-20T18:40:04","modified_gmt":"2018-10-20T16:40:04","slug":"une-elevation-inhabituelle-dalpha-foetoproteine","status":"publish","type":"post","link":"https:\/\/angh.net\/abstracts\/une-elevation-inhabituelle-dalpha-foetoproteine\/","title":{"rendered":"Une \u00e9l\u00e9vation inhabituelle d&rsquo;alpha foetoprot\u00e9ine."},"content":{"rendered":"<p><strong>2012<\/strong><\/p>\n<p><em>S\u00e9verine HOMMEL, Magali PICON, Serge ERLINGER, Didier MATHIEU, Fran\u00e7ois CESSOT (Aix en Provence)<\/em><\/p>\n<p><strong>H\u00e9patologie <\/strong>&#8211; \u00a02012-05-05 &#8211;\u00a0CO &#8211;<\/p>\n<p>________________________________<\/p>\n<p>Nous rapportons le cas d&rsquo;une femme \u00e2g\u00e9e de 32 ans adress\u00e9e pour \u00e9l\u00e9vation isol\u00e9e de l&rsquo;alpha foetoprot\u00e9ine (AFP).<br \/>\nCe dosage a \u00e9t\u00e9 demand\u00e9 par son m\u00e9decin traitant devant des douleurs abdominales \u00e9voluant depuis 1 an associ\u00e9es \u00e0 des ballonnements et \u00e0 une constipation r\u00e9cente, sans notion d&rsquo;alt\u00e9ration de l&rsquo;\u00e9tat g\u00e9n\u00e9ral.Ses ant\u00e9c\u00e9dents sont marqu\u00e9s par une p\u00e9ricardite virale quelques ann\u00e9es auparavant, par un tabagisme actif \u00e0 5PA et par 2 grossesses par voie basse.<br \/>\nLe bilan prescrit par le m\u00e9decin traitant ne retrouve aucun syndrome inflammatoire, le bilan h\u00e9patique est normal mais seul revient \u00e9lev\u00e9 le dosage d&rsquo;AFP \u00e0 51.3 ng.ml(N&lt;10). L&#039;\u00e9chodoppler abdominal est normal. L&#039;examen clinique est sans particularit\u00e9.<br \/>\nIl n&#039;existe aucune prise de toxique, pas de consommation d&#039;alcool et aucune prise m\u00e9dicamenteuse.<br \/>\nLe bilan biologique de controle confirme l&#039;\u00e9l\u00e9vation de l&#039;alpha foetoprot\u00e9ine \u00e0 45.9 ng\/ml, les autres marqueurs tumoraux \u00e9tant n\u00e9gatifs, les s\u00e9rologies virales (VHA, VHE, VHB, VHC) reviennent n\u00e9gatives de m\u00eame que le dosage des B\u00e9taHCG.<br \/>\nLe scanner thoracoabdominopelvien inject\u00e9 n&#039;objective aucune anomalie. Les explorations endoscopiques (FOGD et coloscopie totale) s&#039;av\u00e8rent normales.<br \/>\nLe diagnostic \u00e9voqu\u00e9 devant ces r\u00e9sultats est celui d&#039;\u00e9l\u00e9vation h\u00e9r\u00e9ditaire de l&#039;alpha foetoprot\u00e9ine. Un dosage familial de l&#039;AFP est r\u00e9alis\u00e9, revenant \u00e0 un taux normal chez la soeur et la m\u00e8re de la patiente mais \u00e9lev\u00e9 chez son p\u00e8re (AFP \u00e0 36.3 ng\/ml). Un dosage est effectu\u00e9 par la suite chez l&#039;oncle paternel et la tante paternelle, normal mais impossible chez les grands parents, ceux-ci \u00e9tant d\u00e9c\u00e9d\u00e9s.<br \/>\nCe syndrome d\u00e9crit pour la premi\u00e8re fois en 1983, dont le trait de transmission est autosomique dominant est rare. Seuls 5 cas ont \u00e9t\u00e9 rapport\u00e9s dans la litt\u00e9rature.<br \/>\nIl faut savoir \u00e9voquer ce diagnostic devant toute \u00e9l\u00e9vation isol\u00e9e de l&#039;alpha foetoprot\u00e9ine apr\u00e8s un bilan \u00e9tiologique exhaustif (pour \u00e9liminer notamment les diagnostics diff\u00e9rentiels carcinologiques), le diagnostic \u00e9tant confirm\u00e9 de mani\u00e8re simple par le dosage familial de l&#039;AFP.<br \/>\nCe cas soul\u00e8ve bien \u00e9videmment la question de la pertinence du dosage de l&#039;AFP devant un syndrome abdominal douloureux chronique!<\/p>\n","protected":false},"excerpt":{"rendered":"<p>2012 S\u00e9verine HOMMEL, Magali PICON, Serge ERLINGER, Didier MATHIEU, Fran\u00e7ois CESSOT (Aix en Provence) H\u00e9patologie &#8211; \u00a02012-05-05 &#8211;\u00a0CO &#8211; ________________________________ Nous rapportons le cas d&rsquo;une femme \u00e2g\u00e9e de 32 ans adress\u00e9e pour \u00e9l\u00e9vation isol\u00e9e de l&rsquo;alpha foetoprot\u00e9ine (AFP).
