Centre hospitalier de PERPIGNAN<\/em><\/p>\n<p><strong>H\u00e9patologie <\/strong>&#8211; \u00a02016-05-12 &#8211;\u00a0CO &#8211;<\/p>\n<p>________________________________<\/p>\n<p>Objectif : Comparer la survie \u00e0 un an de patients atteints d\u2019un carcinome h\u00e9patocellulaire (CHC) en situation palliative au stade B selon BCLC 2012, apr\u00e8s traitement combin\u00e9 par chimio-embolisation lipiodol\u00e9e (CEL) et micro-onde h\u00e9patique (MO) versus le traitement recommand\u00e9 (chimio-embolisation seule).<\/p>\n<p>Mat\u00e9riels et m\u00e9thode : Etude r\u00e9trospective de 44 patients atteints d\u2019un CHC au stade BCLC B, trait\u00e9s entre janvier 2012 et mars 2015 soit par traitement associant CEL et MO (31 patients), soit par CEL seule (13 patients). Le nombre de complications s\u00e9v\u00e8re \u00e0 1 mois post proc\u00e9dure (d\u00e9c\u00e8s, insuffisance h\u00e9patique s\u00e9v\u00e8re, cholecystite aigue, embolisation non cibl\u00e9e) et la survie \u00e0 un an \u00e9taient \u00e9tudi\u00e9s.<\/p>\n<p>R\u00e9sultats : Le nombre complications s\u00e9v\u00e8re dans le mois suivant les traitements \u00e9tait de 4 (dont 1 d\u00e9c\u00e8s) en cas de traitement combin\u00e9 et 2 en cas de CEL seule.<br \/>\nLa survie \u00e0 un an chez les patients trait\u00e9s par traitement combin\u00e9 \u00e9tait de 72 % versus 46 % chez les patients trait\u00e9s par CEL seule.  <\/p>\n<p>Conclusion : Le traitement combinant CEL et MO semble augmenter la survie \u00e0 un an chez les patients atteints d\u2019un CHC au stade BCLC B et n\u00e9cessite une \u00e9tude prospective randomis\u00e9e.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>2016 Khemissa Faiza (1) , Guibal Aymerich (2), Farouil Geofffroy (2) , Amouroux Caroline (1), Dahmouni Sofiane (1), Kouaouci Mahdi (1) , Saez Anna (1), Ortiz Marie Carmen (1), Remy Andr\u00e9 Jean (1) (1) service d&rsquo;h\u00e9patogastroenterologie; (2) service de radiologie .
