Ses ant\u00e9c\u00e9dents  sont un syndrome d\u2019apn\u00e9e du sommeil, un accouchement par c\u00e9sarienne, et une ob\u00e9sit\u00e9 morbide (IMC 44) kg\/m2. Une sleeve gastrectomy a \u00e9t\u00e9 r\u00e9alis\u00e9e 2 semaines avant le d\u00e9but des douleurs. A l\u2019admission aux urgences, la douleur est d\u00e9crite comme sus-ombilicale diffuse, tr\u00e8s intense,  et d\u00e9butant 3 jours avant l\u2019admission. Il n\u2019y a pas de trouble de transit, pas de vomissement, pas de fi\u00e8vre. L\u2019examen clinique retrouve un abdomen souple mais tr\u00e8s sensible \u00e0 l\u2019\u00e9tage \u00e9pigastrique. Le bilan biologique montre un syndrome inflammatoire  (leucocytes 8.2 G\/L, CRP 165 mg\/l), une discr\u00e8te an\u00e9mie (11.2 g\/dl). Le bilan h\u00e9patique et la lipase sont normaux. L\u2019\u00e9chographie v\u00e9siculaire ne retrouve pas de lithiase intrav\u00e9siculaire ou de la voie biliaire principale. Le scanner abdomino-pelvien r\u00e9v\u00e8le un pancr\u00e9as caudal augment\u00e9 de taille, h\u00e9t\u00e9rog\u00e8ne, avec une infiltration de la graisse p\u00e9ri-glandulaire et une coul\u00e9e de n\u00e9crose inter-spl\u00e9no-pancr\u00e9atique de 50 mm de diam\u00e8tre ; s\u2019y associe une thrombose portale totale \u00e9tendue au tronc spl\u00e9no-m\u00e9sara\u00efque, \u00e0 la veine m\u00e9sent\u00e9rique sup\u00e9rieure et \u00e0 la veine spl\u00e9nique.<br \/>\nLe diagnostic de pancr\u00e9atite aigu\u00eb n\u00e9crosante CTSI 4\/Balthazar E avec thrombose portale et spl\u00e9nom\u00e9sent\u00e9rique \u00e9tendue  compliquant une chirurgie bariatrique est retenu.<br \/>\nNous d\u00e9taillerons la revue de la litt\u00e9rature concernant cette pathologie et reviendrons sur les m\u00e9canismes \u00e9tiopathog\u00e9niques envisag\u00e9s \u00e0 l\u2019origine  de cette complication post bariatrique.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>2019 Vincent NGUYEN-KHAC , Guillaume VELUT, Florence SKINAZI , H\u00e9l\u00e8ne LABADIE &#8211; Centre hospitalier de Saint Denis, service d\u2019H\u00e9patogastroent\u00e9rologie, 93200 Saint-Denis Gastroent\u00e9rologie &#8211; \u00a02019-05-10 &#8211;\u00a0CC &#8211; ________________________________ Une patiente de 54 ans consulte aux urgences pour des douleurs abdominales \u00e9voluant depuis 72h.
