Introduction
\nL\u2019optimisation du temps de passage en hospitalisation de jour (HDJ) pour la perfusion d\u2019une bioth\u00e9rapie permet de r\u00e9duire la dur\u00e9e d\u2019hospitalisation mais aussi le co\u00fbt de la prise en charge. Un premier travail nous avait permis de r\u00e9duire significativement le temps d\u2019hospitalisation. Le but de cette \u00e9tude est d\u2019\u00e9valuer l\u2019impact d\u2019une validation anticip\u00e9e (VA) la veille de la perfusion sur la dur\u00e9e d\u2019hospitalisation.
\nM\u00e9thodologie
\nDepuis 2016, nous avons mis en place un programme d\u2019\u00e9valuation des pratiques professionnelles dont le but est d\u2019analyser les diff\u00e9rentes \u00e9tapes du parcours du patient en HDJ pour une bioth\u00e9rapie IV : admission \u00e0 l\u2019h\u00f4pital, accueil du patient (installation du patient \u2013 relev\u00e9 des constantes), validation du traitement par le m\u00e9decin, pr\u00e9-m\u00e9dication \u00e9ventuelle, mise en place de la perfusion et surveillance en enfin la sortie administrative.
\nDepuis Janvier 2019, nous effectuons une VA par les infirmi\u00e8res d\u2019\u00e9ducation th\u00e9rapeutique qui consiste \u00e0 t\u00e9l\u00e9phoner au patient la veille de la perfusion (horaire pr\u00e9alablement d\u00e9fini lors de la derni\u00e8re perfusion). Cinq questions sont pos\u00e9es (Fi\u00e8vre > 37.5\u00b0C, douleurs abdominales intenses ou inhabituelles, nombre de selles > 5 ou inhabituelles, prise d\u2019antibiotiques, infection r\u00e9cente) si les voyants \u00ab sont au vert \u00bb et en l\u2019absence d\u2019anomalie biologique, la prescription est valid\u00e9e par le m\u00e9decin. Ainsi, la bioth\u00e9rapie peut \u00eatre pr\u00e9par\u00e9e par la pharmacie d\u00e8s l\u2019arriv\u00e9e \u00ab informatique \u00bb du patient le jour J. Le m\u00e9decin passe quant \u00e0 lui dans la matin\u00e9e pour voir le patient sans contrainte horaire. Nous avons effectu\u00e9 deux types d\u2019analyses : une comparaison entre la p\u00e9riode Avril 2019-Mai 2019 et 2017 concernant le temps global pass\u00e9 \u00e0 l\u2019h\u00f4pital mais aussi d\u00e9termin\u00e9 s\u2019il existait des facteurs pr\u00e9dictifs d\u2019un temps d\u2019hospitalisation r\u00e9duit en fonction de l\u2019heure d\u2019arriv\u00e9e, de la validation ou non la veille, du type de perfusion.
\nR\u00e9sultats
\nSur une p\u00e9riode de 3 mois (Avril \u00e0 juin 2019), nous avons \u00e9tudi\u00e9 les donn\u00e9es de x patients (x maladie de Crohn et Y RCH) hospitalis\u00e9s pour une perfusion d\u2019infliximab (n=) ou de vedolizumab (n=). Parmi ces malades y ( %) n\u2019ont pas \u00e9t\u00e9 valid\u00e9 la veille pour les raisons suivantes : impossibilit\u00e9 de joindre le patient (n=), contre-indication jug\u00e9e par le m\u00e9decin (n=). Le temps global pour une perfusion \u00e9tait de xx, il \u00e9tait de xx min en l\u2019absence de VA et de xx min si VA. Les facteurs associ\u00e9s \u00e0 un temps r\u00e9duit d\u2019hospitalisation \u00e9tait

\nConclusion
\nCe travail montre qu\u2019une validation anticip\u00e9e infirmi\u00e8re-m\u00e9decin permet une r\u00e9duction significative de la dur\u00e9e d\u2019une HDJ et permet aux patients s\u2019ils le souhaitent de manger \u00e0 leur domicile.<\/p>\n","protected":false},"excerpt":{"rendered":"
