A l\u2019entr\u00e9e, la temp\u00e9rature est \u00e0 40\u00b0C en plateau sans instabilit\u00e9 h\u00e9modynamique, l\u2019abdomen est souple et indolore. Les selles sont liquides, sanglantes et glaireuses. Biologiquement, il existe un syndrome inflammatoire avec une hyperleucocytose \u00e0 14 000\/mm\u00b3, une CRP \u00e0 325 mg\/l et une procalcitonine \u00e0 40\u00b5g\/l. Le bilan h\u00e9patique est perturb\u00e9 avec une cytolyse \u00e0 5N et une cholestase (ASAT\/ALAT 157\/232 UI\/l, PAL\/GGT 79\/ 500UI\/l, Bilirubine T\/C : 42\/25\u00b5mol\/l). Les pr\u00e9l\u00e8vements infectieux font d\u00e9couvrir une bact\u00e9ri\u00e9mie \u00e0 bacille gram n\u00e9gatif multisensible. La ponction lombaire est normale, la PCR Covid-19 est n\u00e9gative. Le scanner thoraco-abdomino-pelvien effectu\u00e9 en urgence montre une masse intra h\u00e9patique hypodense pr\u00e9sentant de petites cloisons, sans paroi bien individualis\u00e9e, sans prise de contraste, faisant \u00e9voquer une masse n\u00e9crotique septique ou une l\u00e9sion secondaire. Le diagnostic d\u2019abc\u00e8s h\u00e9patique est retenu et une antibioth\u00e9rapie par C3G \u2013m\u00e9tronidazole est d\u00e9but\u00e9e. Apr\u00e8s quelques jours, la fi\u00e8vre r\u00e9gresse mais les douleurs lombaires se majorent avec l\u2019apparition d\u2019une impotence fonctionnelle des membres inf\u00e9rieurs, un signe de Las\u00e8gue bilat\u00e9ral sans d\u00e9ficit sensitivo-moteur ni trouble sphinct\u00e9rien, sans modification des r\u00e9flexes ost\u00e9o tendineux qui sont conserv\u00e9s.<br \/>\nUne IRM du rachis lombaire est r\u00e9alis\u00e9e en urgence<br \/>\nQuel diagnostic \u00e9voquez-vous ?
