{"id":2384,"date":"2021-10-06T13:54:23","date_gmt":"2021-10-06T11:54:23","guid":{"rendered":"https:\/\/angh.net\/abstracts\/pancreatite-aigue-et-paralysie-faciale-peripherique\/"},"modified":"2021-10-06T13:54:23","modified_gmt":"2021-10-06T11:54:23","slug":"pancreatite-aigue-et-paralysie-faciale-peripherique","status":"publish","type":"post","link":"https:\/\/angh.net\/abstracts\/pancreatite-aigue-et-paralysie-faciale-peripherique\/","title":{"rendered":"Pancr\u00e9atite aig\u00fce et  paralysie faciale p\u00e9riph\u00e9rique"},"content":{"rendered":"<p><strong>2021<\/strong><\/p>\n<p><em>Abel Galland, Marie LAZARETH, Caroline De Kerguenec<br \/>\nService h\u00e9patogastroent\u00e9rologie H\u00f4pital DELAFONTAINE 93200 SAINT DENIS<\/em><\/p>\n<p><strong>Gastroent\u00e9rologie <\/strong>&#8211; \u00a02021-05-17 &#8211;\u00a0CC &#8211;<\/p>\n<p>________________________________<\/p>\n<p>Un patient de 41 ans, aux ant\u00e9c\u00e9dents de remplacement valvulaire aortique par bioproth\u00e8se en 2014 et de diab\u00e8te de type II en rupture de traitement se pr\u00e9sente aux urgences de Delafontaine pour des \u00e9pigastralgies brutales.<br \/>\nA l\u2019arriv\u00e9e, le patient est apyr\u00e9tique, douloureux en \u00e9pigastrique sans d\u00e9fense, il existe une paralysie faciale droite p\u00e9riph\u00e9rique ainsi qu\u2019une parotidite bilat\u00e9rale.<br \/>\n Biologiquement, une lipas\u00e9mie >3N, une \u00e9l\u00e9vation mod\u00e9r\u00e9e des transaminases (ASAT 1,5N ALAT 1,5N) et une cholestase anict\u00e9rique (GGT 8N, PAL 4N),  un d\u00e9s\u00e9quilibre de son diab\u00e8te en rupture de traitement ( Glyc\u00e9mie \u00e0 jeun 32 mmol\/L). Les triglyc\u00e9rides et la calc\u00e9mie corrig\u00e9e d\u2019entr\u00e9e sont normaux. L\u2019\u00e9chographie abdominale r\u00e9alis\u00e9e dans les 24h ne retrouve pas de lithiase v\u00e9siculaire. Le scanner abdomino-pelvien r\u00e9alis\u00e9 \u00e0 72h objective une pancr\u00e9atite aig\u00fce non s\u00e9v\u00e8re CTSI 3.
