Nousbaum, C.Mokhtari, HT.Zougmor\u00e9, P.Pulwermacher<\/em><BR><BR><b> H\u00e9patologie <\/b> &#8211; 06\/05\/2025 &#8211;  Communication orale<\/p>\n<p>Introduction \u2013 But de l\u2019\u00e9tude :<br \/>\nUne \u00e9tude r\u00e9cente (1) pr\u00e9sent\u00e9e \u00e0 l\u2019association fran\u00e7aise de l\u2019\u00e9tude du Foie a montr\u00e9 que le d\u00e9ficit en alpha1antitrypsine est sous-diagnostiqu\u00e9 chez les patients cirrhotiques transplant\u00e9s h\u00e9patiques. Une \u00e9tude de pratique pr\u00e9sent\u00e9e r\u00e9cemment aux JHFOD (poster prim\u00e9), H. Zougmor\u00e9 et al, a \u00e9valu\u00e9 les connaissances des h\u00e9patogastroent\u00e9rologues non universitaires et des m\u00e9decins internistes sur le d\u00e9ficit en alpha1antitrypsine qui sera pr\u00e9sent\u00e9e lors de ce congr\u00e8s et montr\u00e9 un certain degr\u00e9 de m\u00e9connaissance.<br \/>\nNous pr\u00e9sentons ici les premiers r\u00e9sultats de l\u2019\u00e9tude Alpaga2 visant \u00e0 collecter des informations m\u00e9dicales de patients pr\u00e9sentant un d\u00e9ficit en alpha1antitrypsine homo ou h\u00e9t\u00e9rozygote.<\/p>\n<p>M\u00e9thodes :<br \/>\nLes \u00e9l\u00e9ments suivants ont \u00e9t\u00e9 recueillis \u00e0 partir d\u2019un Google form pr\u00e9-\u00e9tabli par un groupe de travail : \u00e2ge, sexe du patient, patient suivi en h\u00e9patogastroent\u00e9rologie ou en m\u00e9decine interne, homozygote ou h\u00e9t\u00e9rozygote, taux d\u2019alpha1antitrypsine, transaminases en x \u00e0 la limite sup\u00e9rieure de la normale, bilirubine totale, albumin\u00e9mie, TP, plaquettes, fibrose (Fibroscan, biopsie), comorbidit\u00e9s (recherche diab\u00e8te, HTA, dyslipid\u00e9mie), co-infection VHB ou VHC, consommation excessive d\u2019alcool, traitement substitutif, patient en attente de transplantation ou transplant\u00e9.<\/p>\n<p>R\u00e9sultats :<br \/>\n44 patients (62 % hommes) \u00e2g\u00e9s de 43 ans (7 \u00e0 70 ans) vus en service d\u2019H\u00e9patogastroent\u00e9rologie pour 93 % (41) et pour 7 % en M\u00e9decine Interne, homozygote pour 54% d\u2019entre eux (24), h\u00e9t\u00e9rozygote pour 20 % d\u2019entre eux avec un taux m\u00e9dian d\u2019alpha1antitrypsine de 0,51 g, extr\u00eame 0,17 \u00e0 37, transaminases \u00e0 1,5 fois la normale (1 \u00e0 2,5) pour les ALAT, bilirubine m\u00e9diane \u00e0 11 \u00b5mol (3,3 \u00e0 50), une albumin\u00e9mie moyenne \u00e0 40 g (31 \u00e0 50), TP \u00e0 85 % (36 \u00e0 97), plaquettes m\u00e9dianes \u00e0 247 000 (107 000 \u00e0 400 000), une cirrhose dans 7 % des cas, fibrose F0F2 dans 80 % des cas (35), avec une biopsie du foie dans 20 % des cas (9), avec une valeur d\u2019\u00e9lastom\u00e9trie moyenne \u00e0 6 (2,6 \u00e0 35), diab\u00e9tique dans 9 % des cas (4), hypertendu dans 23 % des cas, ayant une dyslipid\u00e9mie dans 9 % des cas (4), sans co-infection VHB ; co-infection dans un cas, consommation d\u2019alcool excessive dans 3 % des cas. Parmi les malades cirrhotiques, 9 % avaient une ascite, 7 % une enc\u00e9phalopathie h\u00e9patique. En cas de cirrhose, un Pugh m\u00e9dian \u00e0 11, en attente de transplantation pour 5 % d\u2019entre eux. Un patient \u00e9tait transplant\u00e9. 5 patients recevaient un traitement substitutif enzymatique.
